If you are interested in becoming an authorized carrier for The Match Maker, Inc., please complete and submit our required Carrier Packet to us. Please note you’ll also need to submit a valid Certificate of Insurance listing The Match Maker, Inc. as certificate holder and a copy of your W-9 and FMCSA Operating Authority to us in order to get set up.
